Please read the following instructions regarding the necessary steps to take in applying for a PCRC.

Please bring the following required documents to the counter of Foreign Affairs Division, Changhua County Police Department. Our staffs will be at service for you. (The blank application form is provided at the counter).

• Please complete the application form.
  • Application Form 
• Your original passport or A.R.C. /A.P.R.C. (The original will be returned after verification).
• It costs NT$100 for the first copy, and NT$ 20 for each additional copy with the same content.
• If you cannot apply in person, a proxy can be designated by completing a statement of authorization or take your seal along with the proxy’s original I.D. and above mentioned documents to complete the application.

• You can confirm your application after the submission by calling Foreign Affairs Division (TEL: 886-4-761-5463), or check it through the same web page.【I want to confirm】
• Please bring your original passport or A.R.C./A.P.R.C. when picking up your PCRC (The original will be returned after verification).
• It costs NT$100 for the first copy, and NT$ 20 for each additional copy with the same content.
• If you cannot pick up your certificate in person, a proxy can be designated by completing a statement of authorization or take your seal along with the proxy’s original I.D. and above mentioned documents to complete the application.
  • Statement of Authorization  

1. It usually takes two working days to issue a certificate. However, it may take longer for applications which require an inquiry for criminal records to judicial authorities.
2. According to the Act Governing Issuance of Police Criminal Record Certificates, police records will not be issued to applicants who remain on a wanted list or whose prison sentences haven’t been served or is now serving in prison.
